DOUBLE DISCOUNT
Save $100 if you attend both the Winter Intensive and the Summer 2012 Workshop at Purchase College Registration must be received no later than January 31st for the Summer Workshop for discount to be applied.

*NYC Residents: please mail, email or fax photocopies of 2 forms of proof of residence in the 5 boroughs of NYC, such as a utility bill, driver’s license, etc., within 5 business days of submitting your registration. If such proof is not received within 5 business days, your application will automatically be moved to the end of the list, pending payment or withdrawal. Registration is not guaranteed without payment.

Refund/Cancellation Policy

Tuition refund requests will only be considered with written notice of cancellation. Full tuition refunds will be granted only if written notice is received by December 5, 2011; 50% tuition refunds will be granted only if written notice is received by December 21, 2011. After December 21, 2011, no refunds will be granted. $50 of all fees paid will be non-refundable and considered as a registration fee, regardless of when registration was received.


Registration Contract, Photo Release & Liability Waiver

  1. By registering for the Winter Intensive, I hereby pledge that I am available for the duration of the workshop, and plan to attend the full workshop with no absences, except in cases of unforeseen emergency, under which circumstances I will notify the administration of my absence to the best of my ability.
     
  2. I grant to Dova, Inc., its representatives and employees the right to take photographs of me and my property in connection with the above-identified subject. I authorize Dova, Inc., its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Dova, Inc. may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph.
     
  3. I agree to indemnify and hold harmless Dova, Inc. a.k.a. Doug Varone and Dancers, and any of its faculty or staff members, and 890 Broadway and any of its faculty or staff members from any and all claims, suits, actions, damages and costs of every nature and description arising out of or relating to the use of the 890 Broadway facilities, including but not limited to appurtenances, surrounding grounds, and other campus facilities, transportation in any vehicles operated by Dova, Inc., its staff, volunteers, or interns, and for any injuries sustained, accidents incurred or illnesses contracted by me while a participant of the Doug Varone and Dancers Winter Workshop scheduled to take place between January 9 and 14, 2012. I further agree that I will not hold Dova, Inc. or 890 Broadway responsible for the loss or damage of personal property while in attendance. I agree not to hold Dova, Inc. liable for damages of any kind in the event that 890 Broadway revokes, prior to January 9, 2012, its permit for Dova, Inc. to conduct the workshop. I understand that refund requests will only be considered with written notice of cancellation. Full tuition refunds granted only if written notice is received by December 5, 2011; 50% tuition refunds granted only if written notice is received by December 21, 2011. Refunds will not be granted after December 21, 2011. Teachers, classes and schedule are subject to change.
