REGISTRATION FORM

APPLYING FOR FINANCIAL AID

On the registration form in our brochure, you will notice a line that says “to receive a financial aid/work study application check here.” When you register, check that line and we will send you the application form and requirements. Applications and all required materials are due no later than April 25, 2006. Award notifications should be emailed by May 3, 2006.
 
Requirements include a 5-minute video of your dancing, two (2) letters of recommendation from individuals familiar with your dancing (teachers, artistic directors, etc.), your parents’ IRS 1040 (pages 1 & 2) if you are a dependent*, a personal statement, and Doug Varone and Dancers’ one-page application form.
 
Determination of awards is based on a combination of merit and demonstrated financial need. Because of our many requests for financial aid and our limited scholarship dollars, support is likely to be a partial tuition waiver.  
 
If you are interested in the workshop and wish to apply for aid, you should register to hold your first choice of classes and place in the workshop. Registration requires the first down-payment of $495 and the registration fee of $50. Should you withdraw by May 5, 2005 your tuition will be refunded in full. We will notify financial aid applicants with our decision on financial support by May 3, 2006.
 
* If you are not a dependent, you will need to submit your own IRS 1040 form.
